Please complete one of the below forms. Be sure to print a copy for your records before submitting.
Customer Set Up Form - US
Customers located within the United States, please use this form.
Customer Set Up Form - InternationalCustomers located outside the United States, please use this form.
Step 3: Complete a Customer Set Up Form
If you qualify as a Sales Tax Exempt Business in MN, IA, SD, ND, WI or KY you must provide a signed State Tax Exempt form to Great Northern Equipment to keep on file. Your account will be charged tax until we receive this form.
GNE is registered and authorized to collect and remit sales tax in the following states: IA, KY, MN, ND, SD, and WI. Please refer to the Department of Revenue website links below for further information on sales and use tax and to obtain the proper forms.
